Whistleblowing Audit Form

In a whistleblowing claim you have to show the Employment Tribunal that you made a “qualifying disclosure”. To do this, you must identify the disclosure you made, to whom it was made, the specific section of the Employment Rights Act 1996 (ERA 1996) that your disclosure falls into and the detriment that you have suffered as a result of making that disclosure.  Each disclosure counts. Use this form to make sure that your disclosures qualify for protection and attach it to your claim when you file it with the Employment Tribunal.

Whistleblowing is when you make a disclosure of information that is in the public interest. This is called a protected disclosure. You must not be dismissed or subjected to any detriment because you made a protected disclosure. The Whistleblowing Audit Form form helps you set out your disclosures in line with the specific provisions in the Employment Rights Act 1996.
